Co-authored stuff is really tricky. It's one of the most difficult parts of my volunteer work on the ACM'S Plagiarism and Ethics Committee. Although in principle all authors of a submitted/published paper are supposed to take equal and joint responsibility for the work and its probity (unless someone submitted it with their name on without their permission) there are situations where it seem unreasonable, such as where senior people are the ones at fault and the juniors have no real way of knowing about or dealine with the problem ahead of time.
